Responsibilities:

  • Assist with activities of daily living such as grooming, oral hygiene,
    toileting, bathing/showering, eating, etc.
  • Assist with personal and household chores, such as tidying up room, errands,
    delivering mail and/or newspapers.
  • Transport residents within the community to meals, enrichment activities, and other programs as needed.
  • Ensure resident care plans are reviewed and followed consistently.
  • Observe and monitor residents for behavioral changes and/or adverse reactions to medications or treatment plans. Communicate any changes to clinical staff immediately, as well as document changes thoroughly.
  • Respond to emergency calls, resident/visitor’s requests, and correct
    environment hazards in a timely, appropriate, and safe manner.
    Observe community policies and procedures regarding resident and team member safety, attendance, timeliness, house rules, teamwork, and resident service.
  • Always maintain professionalism and resident confidentiality.
    Adhere to all Infection Prevention, OSHA, fire, and safety regulations
    always, to ensure the safety and well-being of all community residents.
  • Perform all other duties as requested.

Education Requirements:

  • Must possess current CNA or HHA certification.
  • Fulfill and maintain continuing education credits as required by state.
  • Must be willing to obtain and/or maintain current First Aid/CPR certification.

Responsibilities:

  • Evaluates patient’s functional status (muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, work capacity, etc.). Evaluates home environment for hazards or barriers to more independent living. Identifies equipment needs. Participates in the development of the total plan of care.
  • For patients who plan to return to work, the occupational therapist may perform work capacity evaluation and may refer to specialized vocational training in accordance with Organization policy.
  • Develops treatment program and establishes goals for improved function. Communicates plan of care to referring physician and other Organization professionals.
  • May teach new skills or retrain patient in once familiar daily activities that have been lost due to illness or injury, in accordance with Organization policy.
  • May fabricate splints and instruct patient in the use of various types of adaptive equipment to improve function.
  • May train patient in the use of prosthetic and/or orthotic devices. 
  • Maintains appropriate clinical records, clinical notes, and reports to the physician any changes in the patient’s condition. Submits these records in accordance with Organization policy.
  • Maintains contact/communication with other personnel involved in the patient’s care to promote coordinated, efficient care.
  • Attends and contributes to in-services, case conferences, and other meetings as required by Organization policy to ensure coordinated and comprehensive plans of care for the patients of the Organization.
  • Identifies patient and family/caregiver needs for other home health services and refers as appropriate. 
  • Instructs patient’s family/caregiver and other Organization health care personnel in patient’s treatment regime when indicated and appropriate.
  • Supervises certified occupational therapy assistants according to
    organization policy and state regulations.
  • When therapy is the only skilled service, instructs, supervises, and evaluates home health aide care every two (2) weeks.
  • Participates in discharge planning for patient.

Education Requirements:

  • Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association.
  • Registered with the National Registration Examination of the American Occupational Therapy Association.
  • Two years of clinical experience as an occupational therapist.
  • Demonstrates good verbal and written communication, and organization skills.
  • Possess and maintains current CPR Certification.
  • Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements.

Responsibilities:

  • Reviewing new admissions for any special needs or issues that need to be addressed
  • Assisting with patient discharging from a hospital, clinic, or nursing home setting, to return home
  • Coordinating the care of patients with complex needs that require multiple services over time
  • Handling crisis that come up unexpectedly with counseling or therapy
  • Helping patients understand financial and emotional resources available to them

Responsibilities:

  • Evaluate newly admitted patients and develop creative and intensive plans of care.
  • Devise detailed treatment and care plans for each of the patients.
  • Communicate and coordinate with patients’ physicians to ensure a consistent level of care.
  • Treat patients in their homes and teach them how to safely ambulate in their own environment.
  • Exercise independent judgment based on experience to implement effective treatment combinations.
  • Evaluate and report on patients’ progress.
  • Assist administrative staff with billing and reporting inquiries.
  • Comply with federal and state Physical therapy regulations.
  • Work and connect with other therapy and nursing staff to promote excellent communication.
  • Ensure outstanding communication with patients, families, physicians, and teammates.
  • Strive for the best patient experience and outcomes possible.
  • Attend Weekly onsite meetings with Clinical Team.

Physical Therapist Skills:

  • 1+ years’ professional PT experience preferred, not required.
  • Valid MA state Physical Therapist license.

  • CPR and first aid certification required.

Our organization is currently looking for a Registered Nurse in the following areas:

Responsibilities:

  • Do routine home visits
  • Assess new client and do in-take
  • Train and supervise caregivers
  • Apply knowledge of pharmacology, pathophysiology, and basic nursing theory to assess a patient and initiate care.
  • Create a plan of care for a patient that addresses the current needs of the patient and identifies future needs.
  • Communicate status of patient to physicians, family members, and other team members.
  • Provide hands-on care to patients (e.g., hygiene, bathing, dressing wounds) in accordance with physician’s orders and established standards of nursing practice.
  • Report changes in patient’s condition to physicians or other team members.
  • Perform diagnostic and therapeutic nursing procedures as needed (e.g.,taking vital signs, suctioning, flushing IV lines).
  • Assess patients for injuries, record patient information in medical files, perform nursing calculations (e.g., calculate IV drip rates), prepare syringes for medication administration, administer medication by injection or infusion through an IV line or inhalation therapy.

Weekly schedule:

  • Monday to Friday
  • On Call and Weekends (Part Time RNs)

  • Keeping up-to-date with advancements in the field by attending conferences, conducting research, and expanding professional and academic networks.
  • Educating patients and their families on speech disorders and available therapies.
  • Taking detailed notes on patient history and progress.
  • Providing mentoring and workshops to speech therapy students.
  • Evaluating patient progress and implementing changes to therapy programs as required.
